Ordering a PET Scan in Ontario
Ontario continues to approve new indications for PET scan use. Below is a brief overview of the streamlined ordering process, to help you put PET scans to work for your patients.
How do I order a PET scan?
For Ontario patients receiving a PET scan within Ontario:
1. Review the Indications to confirm the patient’s eligibility.
2. Download and complete the relevant form.
3. Gather supporting documentation as specified in the form.
4. Submit the completed form by fax to your PET centre (full list of locations here) or PET Scans Ontario, as instructed on the form.

Apply for a PSMA PET scan for case-by-case review through the PET Access Program, when the PET scan is a problem-solving tool, where the confirmation of the site of disease and/or disease extent may impact clinical management over and above the information provided by conventional imaging.
